This is a tricky one. Since you have the enemy suit, you can consider NT. But 3NT might be a bit of a stretch, considering that East might have as few as 10HCP. Partner might interpret 2NT as weak. And although you have 4 of them, your Spades are weak. You can "pass for penalties", converting the double in a penalty bid, wiht a good chance of getting them down.
